This process only works if the final appointment is in the future.

• Find the patient by going to your ‘Patients’ tab and searching for their EMIS number

• If there is more than one Episode of Care, select the most recent, go to Questionnaires tab and select ‘Assign Questionnaire’

• Click on the drop down next to ‘Select appointment’ and choose from the drop- down list

• Select GAD7 and PHǪ9 from the ‘required questionnaires’ section

• You can select a due date if you wish, but this is not required.
• Then select ‘Assign Questionnaire’
