Completing Assessments
As a patient, you will be asked to complete wellbeing assessments at various stages of your treatment. These assessments help us monitor your progress and demonstrate our effectiveness to NHS commissioners. You will typically be asked to complete assessments:
- During registration.
- After 6 months of treatment.
- At the end of your treatment.
If you are undergoing a course of therapy, your therapist or clinician may request that you complete assessments more frequently to better understand your wellbeing.
1: Receive Notification of New Assessments
- Booking Platform Notification:
- If an assessment is assigned to you, you will receive a notification through the booking platform before your next appointment.
- Pop-Up Reminder:
- A pop-up box will appear on your homepage reminding you to complete the assessment.

2: Access Assessments
You have several options to access your assessments:
- Recent Assessments:
- On your homepage, navigate to the 'Recent Assessments' section to view completed assessments.
- Take Assessments:
- On the left-hand side of your homepage, click on 'Take Assessments' to access any available assessments you wish to complete.
- Assessments Tab:
- Click on the 'Assessments' tab located at the top of your login screen to view all assessments.


3: Complete the Assessment
- Select an Assessment:
- Choose the assessment you need to complete from the list provided.
- Follow Instructions:
- Carefully read each question and provide your responses.
- Submit Assessment:
- After completing all questions, click 'Submit' to finalise your assessment.
4: View Completed Assessment
- Once submitted, your completed assessment will appear under 'Recent Assessments' on your homepage, which you can review at any time.
